DIFFUSELY INTERSTITIAL MARKING, THICKENED inter-lobar fissure, Kerley-B lines

Cephalization of vessels

Only valid on upright film

Thickened inter-lobar fissure

Peri-hilar crowding

45 yo male smoker, with sharp pleuritic chest and back pain

Pleural reflection (you see a line that does not belong to a rib.)

This is PNEUMOTHORAX

TIPS SUSPECT PNEUMOTHORAX - Can take Inspiratory view BUT ALSO Expiratory view (condense lung tissue down making it easier to see the pneumothorax.)

ANOTHER TRICK IS – INVERT THE IMAGE - HOT LIGHT OR SPOT LAMP on the desk. Take the hot light, take a look all around the periphery. Highlights lines, highlights pulmonary vasculature, helps detect suddle pneumothoracis.
